for Patrons Under 18 Years of Age

The Internet is a vast online resource for limitless information. Most of this information is useful and interesting, and parents can feel very comfortable having their children learn from the Internet.

However, the Thompson Public Library does not monitor nor censor the information accessed on the Internet and cannot be held responsible for its content. Parents or guardians, not the Thompson Public Library nor its staff, are responsible for Internet information selected and/or accessed by their children.

Please fill out this form and return it the Thompson Public Library. Without it, your child will not be allowed Internet access. Users under 18 years of age are required to have this signed permission slip on file.
